The administrator of our multispecialty clinic has successfully negotiated two of the three managed-care contracts and has now been charged with recruiting additional providers to meet some of the patient-care needs of the clinic. In reviewing patient demographics, it is apparent that the clinic needs a pediatrician and an obstetrician because the practice has taken on more female patients of childbearing age, and current staff physicians either do not treat children under 12 or do not carry malpractice insurance for obstetrical care.
As part of this recruitment process, the administrator must consider such factors as compensation, productivity models, and educational levels. Additionally, maternity patients require more frequent monitoring and care. To keep babies delivered into the practice, pediatric services must be added, and non-physician providers must be considered. Would nurse practitioners or physician assistants be qualified and capable of handling some of the treatment needs of these patients? What must the practice offer in the way of a compensation package for these non-physician providers? What are the associated liability costs, and how will patients feel about seeing non-physician providers instead of physicians?
Hospitals, Management, and Regulation of Costs
The next challenge relates to hospital privileges for the pediatrician, the maternal and child nurse practitioner, and the obstetrician the administrator was able to successfully recruit into the practice. Current providers were previously affiliated with one hospital in the community, but this hospital does not currently offer maternity services or pediatric care. However, a local children’s hospital and two community hospitals do offer maternity services and pediatric care, so the administrator must now determine which of these facilities with which to affiliate the new recruits.
The administrator must assess the type of equipment, technology, medical record systems, and associated costs to the patients at these hospitals and address the following questions:
• Are these hospitals participating providers for Medicaid?
• How many other providers are available for the on-call pool system in these hospitals?
• What other aspects of hospital affiliation must be considered?
